With all Cohort Two clinics having signed their clinic agreements with Briljent, coaching calls and site visits are now being scheduled with the PCMH team. If you haven’t returned your signed Memorandum of Understanding (MOU) to IDHW, please do so as soon as possible to OHPI@dhw.idaho.gov.
The PCMH team will conduct a webinar on change management for Cohort Two participating clinics on April 19th. Cohort Two clinics can register for the webinar series by going to the PCMH portal. In addition, the Cohort Two Learning Collaborative is scheduled for June 27th and 28th in Boise. Watch this space for more information about the collaborative agenda and activities or go to the ‘Events’ tab after logging into the PCMH portal.
After results of a Cohort One program evaluation survey were analyzed, the PCMH team used the information to make modifications to the PCMH technical assistance curriculum to make coaching more effective for Cohort Two.Specific findings included scheduling site visits to be conducted earlier inthe cohort year, providing separate learning tracks based on levels of PCMH knowledge and experience, and increasing the PCMH coaches’ understanding of primary care within rural Idaho communities. NCQA 2017 PCMH standards will be announced April 3rd.We’ll be releasing more information on how this impacts participating SHIP clinics as soon as it is available.
