The application for SHIP Cohort One Clinics to become designated as virtual PCMHs is now open! The virtual PCMH is Idaho’s unique approach to expanding healthcare access in medically underserved communities. It is the goal of SHIP to create virtual PCMHs by helping primary care practices improve healthcare access in rural and underserved areas through Community Health Emergency Medical Services (CHEMS), Community Health Worker (CHW), and telehealth programs. SHIP Cohort One clinics with at least one of these programs may apply for designation as a virtual PCMH. Designated clinics will receive a $2,500 reimbursement to help offset program costs. A new upcoming opportunity for Cohort One clinics is a competitive grant to develop telehealth capabilities or expand existing ones. Grant funds may be used for most telehealth-related expenditures except equipment and the direct delivery of services. The grant application became available on March 13th and a technical assistance webinar will be held on March 23rd. More information on the application and webinar will be forthcoming.
Currently, both of these opportunities extend only to Cohort One clinics. The virtual PCMH application and telehealth grant opportunity for Cohort Two clinics will occur later in 2017.
