SHIP Cohort Two clinics are moving into full-swing with their transformation plans with the start of in-person site visits that began in late April. The visits take place among PCMH team coaches (HMA), Public Health District staff, and clinic staff. The visits allow clinics to communicate unique elements of their practices and their communities; they allow PCMH coaches to become acquainted with the clinic team and the practice culture, and to build on the working relationships they’ve established with the PHD SHIP QI staff. During the site visit the PCMH coaches will help clinic teams develop their transformation plans and discuss processes and priorities for PCMH transformation.
Save the Date! Another opportunity for Cohort Two clinics will take place in June at the PCMH Learning Collaborative (LC). The LC will create a rich learning environment for networking and sharing; it will provide stories from the field as clinic teams talk about their journeys toward a patient centered medical home; and discussions will take place about key principles supporting a patient centered medical home. Clinics will leave with tools and tips for their transformation journey. The Learning Collaborative will take place June 27 and 28 at Boise at Boise State University.
