Congratulations to Cohort Two clinics for a successful Learning Collaborative! One hundred sixty-five people from all 56 Cohort Two participating clinics attended along with members of the State Evaluator team, Medicaid, Healthy Connections, Qualis, and IHDE. Over the two-day event we discussed the vision and progress for Idaho’s PCMH transformation.
Day one started with Coeur d’Alene Pediatrics and St. Mary’s Clinic engaging the crowd with stories of their successful transformations. The message from both: transformation is a journey, not a destination. Break-out sessions included Becoming a PCMH for clinics that are new to the PCMH model; and Care Management & Behavioral Health Integration for clinics that are more experienced. Day one ended with a World Café exercise that had clinics working together on a variety of topics including value-based payments, access to care, and care management.
Day two began with an interesting discussion about how to lead change in an organization going through PCMH transformation. Break-out sessions included Risk Stratification and Population Health Management, Steps to Team Based Care, and Patient Engagement. The event closed with a presentation by HMA on embedding quality improvement which included a talk by Valley Medical Center about their approach to driving outcomes.
Initial evaluations were positive – it appears that everyone took away something they could work on. Presentation slides are available on the portal.
