SHIP is a CMMI grant that is anticipated to last from February 2015 to February 2019. The grant was applied for and awarded to the Idaho Department of Health and Welfare (DHW). SHIP is a huge system transformation effort that includes many entities from DHW and the community.
SHIP Staff: As part of the grant, eight full time staff positions have been allocated. The team members salaries are paid with the federal grant dollars (<10% of the budget). The staff are DHW employees within the Office of Healthcare Policy Initiatives which is housed within the Director’s Office.
SHIP Team: DHW staff members come from across the whole agency. These team members are subject matter experts and leaders in various areas including Medicaid, Public Health, Welfare, and Behavioral Health.
Contractors: SHIP utilizes multiple contractors to assist with various components of the project. These contractors provide subject matter expertise, skills and resources needed to implement the change and advance the project goals.
Regional Health Collaborative: As part of the Public Health Districts, Regional Health Collaboratives are responsible for supporting patient-centered medical homes providers and establishing the medical neighborhood. These locally based entities contracted with SHIP to support this critical role.
