The Statewide Healthcare Innovation Plan (SHIP) is a CMMI grant that was anticipated to last from February 2015 to February 2019. The grant was applied for and awarded to the Idaho Department of Health and Welfare (DHW). SHIP was a huge system transformation effort including many entities from the Idaho Department of Health and Welfare (IDHW) and the community.
SHIP Staff: As part of the grant, eight full time staff positions were allocated to the SHIP. The team members salaries were paid with the SHIP's grant dollars (<10% of the budget). The staff were IDHW employees within the Office of Healthcare Policy Initiatives housed within the Director’s Office.
SHIP Team: IDHW staff members came from across the whole agency. These team members are subject matter experts and leaders in various areas including Medicaid, Public Health, Welfare, and Behavioral Health.
Contractors: The SHIP utilized multiple contractors who assisted with various components of the project. These contractors provided subject matter expertise, skills and resources needed to implement the change and advance the project goals.
Regional Health Collaborative: As part of the Public Health Districts' Regional Health Collaboratives were responsible for supporting patient-centered medical home providers and establishing the medical neighborhood. These locally based entities contracted with the SHIP to support this critical role.
