SHIP Project Healthcare Survey

Context:

Evaluation of patient involvement in their own healthcare is a required piece of the SHIP grant. The information from this study will be used to inform policy makers at the state level about how to improve the quality of healthcare throughout the state. Study information will be kept confidential.

She is responsible for interviews with Community Health
Workers, Community Health Emergency Medical Service providers, Regional Collaborative co-chairs, and production of the PCMH sustainability video series.

 

 

The goals of the study are to help researchers:
Obtain patient feedback about the healthcare clinic they attend
Understand changes in patient involvement in their own healthcare
Help improve healthcare quality

The objectives of the patient interview questions are to:
A. Understand how patients participate in their own healthcare
B. Determine how clinic healthcare teams can better meet patient needs
C. Identify roadblocks to quality healthcare

All information from the study will be kept confidential. Survey will be conducted at the patient’s convenience, either in-person or over the phone. Patients who participate in the study will receive a $50 reimbursement for participating.
