The goal of SHIP’s virtual Patient-Centered Medical Home (PCMH) is to improve access to primary care services in Idaho’s rural and underserved communities through Community Health Emergency Medical Services (CHEMS), Community Health Workers (CHW), and telehealth programs. These programs extend the reach and capacity of primary care services throughout the state.
CHEMS program development is actively underway
with licensed paramedics participating in the community paramedic program at Idaho State University. Paramedics participating in this year-long program agree to collect and report data to test the CHEMS concept against the Triple Aim. Paramedics from Ada, Bonner, Bingham, and Canyon Counties completed the course in December 2016 and we encourage PCMHs and stakeholders to partner with them to implement this innovative model. EMS agencies from Ada, Bonner, Bonneville, Boundary, Canyon, Payette, Shoshone, Valley, and Counties began the 2017 program this month.
The CHW training program began January 10th and is being delivered live online one evening per week for 16 weeks also through Idaho State University. A maximum of 25 participants may participate in the program at no charge when they agree to participate in the SHIP initiative and submit quarterly data to IDHW.
The telehealth initiative includes a six-part webinar training series to assist SHIP cohort clinics in building the capacity, knowledge, and expertise to successfully develop and implement a telehealth program. Topics in the series include: conducting a demand analysis and readiness assessment, billing and coding for services, equipment selection, program development, and evaluation. All webinars are recorded and available on the SHIP telehealth webpage. Watch for information in February regarding a SHIP grant program for cohort clinics to seek funding for telehealth program development in their clinics.
The virtual PCMH initiative also includes a $2,500 reimbursement for SHIP cohort clinics that successfully adopt any one of the CHEMS, CHW, or telehealth programs. The application for virtual PCMH designation will be available to SHIP Cohort One clinics in February 2017.
