In December, IDHW selected the second cohort of 55 clinics to
participate in SHIP. Beginning February 1, 2017, these clinics will join the
first cohort of 55 clinics representing small and large clinics in urban,
rural, and frontier communities throughout the state. Like Cohort One, these
Cohort Two clinics are comprised of community health centers, private
practices, rural health centers, and health system/ hospital-owned clinics. A
few of the Cohort Two clinics include a non-profit children’s clinic, a
free/charitable clinic, and one behavioral health integrated family practice.
Seventeen of the selected clinics are nationally accredited or recognized as
PCMH. These clinics will receive training based on their higher level of transformation
experience and will have the opportunity to mentor clinics within the cohort
and their region that have less experience with their PCMH transformation
In January, Cohort Two clinics will receive additional information
regarding their participation with SHIP from Briljent, SHIP’s PCMH technical
assistance contractor, as well as from the Idaho Health Data Exchange (IHDE).
