The Idaho Department of Health and Welfare is currently accepting SHIP Interest Surveys for clinics interested in participating in SHIP Cohort Three. The survey will be available through Friday, September 15, 2017 to any primary care clinic that is not currently participating in SHIP. The CHW training through ISU will be offering two core courses in fall 2017. One course will be live-online and one will be offered in-person in the North Central Health District. These courses will start August 22nd and run for 13 weeks. The online course will be open to a maximum of 25 students and the in-person course will have a maximum of 20. Patient engagement. Knowing more about it and documenting the patient experience is a universal goal. It’s key to developing the Patient Centered Medical Home; it’s critical to ensuring the best care while evolving to a value-based payment environment; and it’s a requirement of the SHIP grant. But obtaining this valuable patient information isn’t easy. To try to obtain the data, the SHIP State Evaluator research team has developed a short survey for SHIP clinic patients.
The research team is asking SHIP clinics to provide them with a list of 15-20 patients they think might be willing to participate in one-on-one interviews about their own definitions of self-care, behaviors they feel they may need to change, and their readiness to become engaged with their healthcare team to take better care of their health.
The research team will recruit patient participants from the list provided and will conduct the short interviews. Analysis and reporting of results will also be conducted by the research team who will then provide each clinic with its anonymous aggregated results on patient engagement in the clinic’s Patient Centered Medical Home.
It’s important to know that all informed consent requirements will be met and HIPAA data will not be collected. Patients will also receive a reimbursement for their participation.
